On or about June 27, 2018, at the left front corner of the house, the gasoline can used to refuel the air compressor motor was not closed in that the pour nozzle was not capped, exposing employees to a fire or explosion hazard from evaporating gasoline vapors.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.    On or about June 27, 2018, at the left rear corner of the roof, the rails of the access ladder did not extend 3 feet above the edge of the roof, exposing employees to a 9 feet fall hazard.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  a.    On or about June 27, 2018, at the front left corner of the house, the Werner 8-foot aluminum stepladder had been damaged at the foot of the right rail, and the ladder had not been removed from service, exposing employees to the hazard of falling from the ladder.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.   Employees working at elevations greater than 6 feet from the ground or lower levels while performing commercial framing are not being protected from fall hazards by a fall protection system.  The employer fails to assure effective fall protection against falls as required by the standard as most recently demonstrated on:   1.    On or about June 27, 2018, on the roof sheathing and trusses of the 6:12 pitch roof, employees installing truss framing were exposed to fall hazards at heights of 10 feet to 20 feet down through the trusses in that they were not protected by a fall protection system.  MJC Construction Services, Inc., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1308785, citation number 1, item number 1 and was affirmed as a final order on June 1, 2018, with respect to a workplace located at 119 Palisade Drive, Saint Augustine, Florida.

29 CFR 1926.503(a)(1): A training program was not provided to each employee who might be exposed to fall hazards, to enable that employee to recognize the hazards of falling, and be trained in the procedures to be followed to minimize these hazards:  a.    On or about June 27, 2018, employees working at elevations were not trained to correctly assemble fall protection equipment in that they did not properly anchor their personal fall arrest systems, exposing themselves to 20 feet fall hazards
