Worksite at 721 W Britton Rd. Oklahoma City, OK 73114. On or about June 27, 2018, employees were observed installing canopy material at a height of more than 6 feet without the use of fall protection. Employees were exposed to fall hazards. .

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: South canopy. On or about June 27, 2018, the portable ladder used to access the canopy did not extend at least 3 feet above the landing surface, exposing the employee to a fall hazard.
