29 CFR  1926.1053(b)(1):  Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     On or about February 8, 2022, at a north porch roof,  the employer did not ensure an employee used a portable ladder that is secured at its top to a rigid support when the ladder side rails did not extend at least 3 feet above the upper landing surface.  This exposed the employee to a fall hazard.  Mid-America Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1326570, citation number 1, item number 2 and was affirmed as a final order on July 28, 2018, with respect to a workplace located at 721 W. Britton Road, Oklahoma City, OK 73114.
