29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about September 21, 2022, an employee performing construction work activities from the roof of commercial building was not in use of fall protection. This exposed the employee to a fall hazard greater than six (6) feet to the concrete ground below. The Mid America Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926. 501(b)(10), which was contained in OSHA inspection number 1404706, citation number 1, item number 2 and was affirmed as a final order on 08-30-2019, with respect to a workplace located at 4025 NW 63rd Street in Oklahoma City, Oklahoma 73116 . 29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about September 21, 2022, an employee was working from a roof and was not protected from falling through a hole in the roof. This exposed the employee to a fall hazard greater than six (6) feet to the concrete ground below.
