A) Studer-Obringer, Inc. on or about July 9, 2018 at the address of 108 Whittlesey, Norwalk, OH: The employer did not ensure a 8ft x 4ft scaffold platform was securely attached to the forks while employees were performing bricklaying activities at a height of 20 feet above the ground below.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    b) Studer-Obringer, Inc. on or about July 9, 2018 at the address of 108 Whittlesey, Norwalk, OH: The employer did not ensure employees were protected from a fall while working from a scaffold platform.  The employee was exposed to a 10ft fall to the level below while installing fascia material at the roof's eaves.
