What this inspection record means

OSHA opens inspections for many reasons — routine scheduling under a national or local emphasis program, an employee complaint or referral, or a follow-up after a reported injury. Opening or conducting an inspection is not itself an allegation or a finding that this employer broke any rule; any findings appear as the citations listed below, and citations can be contested, reduced, or withdrawn.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person: (a) MCC, Ltd at 2131 Lawrence, Denver, CO 80205: On and before June 22, 2018, the employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person. The employer participated in the design of the plan to lift 18 foot long, 10 inch wide metal planks, weighing about 80 pounds each, up six flights of stairs to the roof. The employer did not ensure that a competent person inspected the site to ensure that safe means and methods were used to lift these materials, that workers stayed away from the drop zone after manually lifting the metal planks, and that workers wore appropriate personal protective equipment. This condition exposed workers to the hazard of being struck-by falling objects.

Workers lifted metal planks up six flights of stairs by holding the planks over the edge of the stair rails, and manually lifting the planks up in between the flights of stairs. The planks were 18 feet long, 10 inches wide, and weighed an estimated 80 pounds each, with multiple planks being hoisted up over a period of 8 hours. The workers were not trained to stay clear of the drop zone after lifting.
