OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  (a) on or about 6/21/2018, in the parking lot of Giddings Boat Works, in Charleston, Oregon, a worker was exposed to crushing hazards when a Forklift was observed being operated in an unsafe manner in that the operator was not wearing a seatbelt.

29 CFR 1915.56(b)(2): Welding cable was not free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected.  (a) aboard the mv Evie Grace, on or about 6/21/2018, and at times prior thereto, welders were exposed to electrical shock hazards, in that welding leads contained repairs approximately 5 feet from the working end.
