29 CFR 1910.219(d)(1): Pulleys with parts seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): a) Supermax Mill: On or about July 12, 2018, and at times prior, workers were exposed to caught in hazards where the left and right belt guards were missing at the variable speed cone pulleys. Abatement Note: The employer completed abatement on July 13, 2018. Certification provided via e-mail.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) On or about July 12, 2018, and at times prior, two managers were exposed to performing a lockout procedure out of sequence and without the requisite specific information required to be included in a lockout procedure. Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).
