29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about February 14, 2022, and at times prior, employees used a Piranha Ironworker punch machine at the production area. The guard cover on the point of operation of the punch machine was missing. This condition exposed employees to possible serious caught in hazards while using the punch machine. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.305(j)(4)(v): The disconnecting means was not readily accessible: a) On or about February 14, 2022, and at times prior, a disconnecting mean switch was not readily accessible to the employees. The disconnect means switch was used to power to their machines and equipment in the production area and was blocked with parts and material. This condition exposed the employees to serious contact with electrical current hazards in the event of an emergency. 29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: a) On or about February 14, 2022, and at times prior, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with their product identifier and words. Several containers were used to store used chemicals and were not properly labeled. This condition potentially exposed employees to products that contain hazardous chemicals. b) On or about February 14, 2022, and at times prior, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with their product identifier and words. Two spray containers were used to store anti-splatter and Windex chemicals and were not properly labeled. This condition potentially exposed employees to products that contain hazardous chemicals.
