29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers were not provided, used, and maintained in a sanitary and reliable condition whenever it was necessary by reason or hazards of processes of environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  (a) Silver Plating line 6 - On or about 6/30/18, a hose came off and there was a spill of about 30-40 gallons of silver cyanide solution.  Employees cleaned up the silver cyanide solution (which contains hazardous chemicals such as, but not limited to, potassium cyanide and potassium silver cyanide.)  Two employees contacted the silver cyanide solution and got a rash on both arms.   The employer did not provide employees with appropriate arm protection.  (b) Silver Plating line 6 - On or about 6/30/18, a hose came off and there was a spill of about 30-40 gallons of silver cyanide solution.   Employees cleaned up the silver cyanide solution (which contains hazardous chemicals such as, but not limited to, potassium cyanide and potassium silver cyanide.)  An employee got the silver cyanide solution on his pants.  The employer did not provide employees with appropriate torso protection.  ABATEMENT CERTIFICATION REQUIRED

The location and availability of the written Hazard Communication Program, list(s) of hazardous chemicals and Safety Data Sheets.       Employee training must include at least:  1.     Methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area. 2.     The physical and health hazards of the chemicals in the work area.  3.     The measures employees can take to protect themselves such as specific procedures, appropriate work practices, emergency procedures and personal protective equipment to be used.  4.     The details of the employers Hazard Communication Program including an explanation of labeling systems,  Safety Data Sheets and how employees can obtain and use the appropriate hazard information.   ABATEMENT DOCUMENTATION REQUIRED    Precious Plate was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(h)(1), which was contained in OSHA inspection number 1074250 citation number 1, item number 4 and was affirmed as a final order on 02/01/2016, with respect to a workplace located at 2124 Liberty Drive Niagara Falls, NY 14304.
