29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a)  On or about 02/09/2023, in the Maintenance Area; employees were exposed to flying debris and rotating parts hazards when using a table to operate Makita router Model RF1101 Serial #93481A without guarding the blade and baring. Employees use the router to shave off, square, and notch out sides and edges of plastic materials.   b) On or about 02/09/2023, in the Maintenance Area; employees were exposed to flying debris and rotating parts hazards when operating the Frejoth lathe Model LC1340G without guarding the chuck. Employees use the lathe to resurface wheels, fabricate hose barbs and sanding down other materials.   ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a)  On or about 02/02/2023, in the Maintenance Area; When using a Makita router Model RF1101 Serial #93481A attached to a table, employee suffered a fingertip amputation when the material he was holding slide across the table pulling his fingers across an unguarded blade. The router and table did not have proper guarding to prevent access to the point of operation. The employee used the router to shave off, square, and notch out sides and edges of a plastic lid.    ABATEMENT DOCUMENTATION REQUIRED
