29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    a) On or about 06/17/19 at the upper mezzanine above Lines 10-13; employees were exposed to a fall hazard of eleven feet when the top rail and mid rail of the guardrail system on the northwest corner of this area consisted of chains between the post frames. 29 CFR 1910.28(b)(3)(iv): Each employee was not protected from falling into the ladderway platform hole at the entrance to the hole by a self-closing gate or offset.  a) On or about 06/17/19 in the Waste Water Treatment area; employees were exposed to a fall hazard of seven feet when the ladderway platform entrance opening on the Scrubber Liquid platform was protected with two chains.   b) On or about 06/17/19 in the Waste Water Treatment area; employees were exposed to a fall hazard of 53 inches when the ladderway platform entrance on the H filter press platform was protected with two chains.
