29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about July 16, 2018; on the jobsite, the employer allowed employees to perform framing activities without adequate fall protection at a height of over 15 feet.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least three (3) feet above the upper landing surface being accessed.  On or about July 16, 2018, employees engaged in framing activities were exposed to fall hazards in excess of 15 feet while using a aluminum extension ladder that did not extend three (3) feet above the landing.

29 CFR 1926.102(a)(1):  The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles:  On or about July 16, 2018, on the jobsite, the employer allowed employees to perform activities without eye protection  while performing framing work on a single family dwelling.  Jose Alfredo Ramirez was previously cited for violation of  this Occupational Safety and Health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number #1195142, citation number one, item number one b and was affirmed as a final order on 05/26/2017, with respect to a workplace located at 42nd Street, Snyder, Texas 79549; inspection number #1273802, citation number 2, item number 1 and was affirmed as a final order on 08/08/2018, with respect to a workplace located at 1111 30th Street,, Lubbock, Texas 79411.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about July 16, 2018, at the jobsite, the employer allowed employees to work without adequate fall protection while performing framing work on a single story residential home at a height of approximately 15 feet.  Jose Alfredo Ramirez was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 1926.501(b)(13), which was contained OSHA inspection number #770763, citation number 1, item number 3, and was affirmed as a final order on 05/31/2015. with respect to a workplace located at 3017 111th Street, Lubbock, TX; which was contained in OSHA inspection number #1195142, citation number 2, item number 2, and was affirmed as a final order on 05/26/2017, with respect to a workplace located at 42nd St. Snyder, TX.
