29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Nelson Stud Welding, Inc. - Elyria, Ohio: On or about June 20, 2018, the employer did not ensure that all employees involved in group maintenance activities applied energy control devices to the electrical disconnect of the Formax FX75 machine during the removal of a drive chain. Three employees were performing this maintenance task under two lock out devices.
