29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a- 101 - 115 Meridian Lane, Sewell NJ: Company worksite workers, not using eye and face protection, were exposed to struck by injuries when using pneumatic nailers, electric circular saws, etc. to perform sheathing installation and other related activities at the worksite. Condition observed on or about 08/04/18. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii) a- 101-115 Meridian Lane, Sewell NJ: Company worksite workers were exposed to fall hazards in excess of 25 feet to the ground surface below when performing roof sheathing installation and other related activities while working/walking from atop pitched roof trusses and while walking/working from newly installed roof sheathing set on top of pitched roof trusses. During each of these activities these workers were not using any fall protection. This condition was noted on or about 08/04/18. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided: a- 101 - 115 Meridian Lane, Sewell NJ: Employees, not using fall protection, were exposed to fall hazards in excess of 25 feet to the ground surface below, when walking and working from the top of pitched roof trusses as well as walking/working from plywood that was installed onto the top of the pitched roof trusses. Condition observed on or about 08/04/18. REF Construction, Inc. was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1253158, citation number 1, item number 1 and was affirmed as a final order on 10/10/17, with respect to a workplace located at Star Drive Site, A010 A- D , Eastampton Village Townhomes, Eastampton NJ. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
