29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork: a) 95 Hornbean Ave., Woolwich, NJ: On or about Feb 27, 2019 a REF Construction Inc employee was working from a plywood scaffold platform rested on the forks of a Pettibone Rough Terrain Fork Lift. The platform was not secured to the forks, exposing the employee to a fall hazard of over 20 feet to the ground below.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. a) 95 Hornbean Ave., Woolwich, NJ: On or about Feb 27, 2019 a REF Construction Inc employee was working from a plywood scaffold platform rested on the forks of a Pettibone Rough Terrain Fork Lift. There was no guard rail, fall restraint, or personal fall arrest system in use, exposing the employee to a fall hazard of over 20 feet to the ground below.
