29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On or about October 6, 2017, during the installation of roofing material at the job site located at 1255 Longshadow Ave NW, North Canton, Ohio 44720; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about August 6, 2018 at the job site located at 1255 Longshadow Ave NW, North Canton, Ohio 44720; during the installation of roof sheeting material on a residential roof, the employer did not ensure portable ladders extend 3 feet above the upper landing surface.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about August 6, 2018, at a work-site located at 1255 Longshadow Ave NW, North Canton, Ohio 44720; during the installation of roof sheeting material, the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 10.1 feet.     The David Troyer Construction LTD was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(1) Duty to have fall protection), which was contained in OSHA inspection number 1148509, citation number 01, item number 01 and was affirmed as a final order on 6/27/2016, with respect to a workplace located at 779 White Pond Dr, Akron, Ohio 44320.

The David Troyer Construction LTD was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(1) Duty to have fall protection), which was contained in OSHA inspection number 1148509, citation number 01, item number 02 and was affirmed as a final order on 6/27/2016, with respect to a workplace located at 779 White Pond Dr, Akron, Ohio 44320.
