29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) New townhome construction located on Lot #112, North side of structure: On or about 08/07/2018, and at times prior, the employer exposed employees to ejection/fall hazards of approximately 28-feet, 8-inches to the lower level, in that a fall arrest system was not used while installing cornice boards in an aerial lift basket.

29 CFR 1926.454(a)(2): Employees have not received training in the correct procedures for dealing with electrical hazards and for erecting, maintaining, disassembling the fall protection systems and falling object protection systems being used: (a) New townhome construction located on Lot #112, North side of structure: On or about 08/07/2018, and at times prior, the employer failed to train employees regarding fall protection while using aerial lift equipment, in that the proper lanyards needed to work in an aerial lift was not provided or used in the basket.
