29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.  On August 8th 2018, at a residential job site located at 8215 Northeast 90th Street Kansas City, Missouri 64157, employees were exposed to an electrical shock hazard from the use and operation of a Rolair Systems electric air compressor with damaged insulation and exposed wires.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On August 8th 2018, at a residential job site located at 8215 Northeast 90th Street Kansas City, Missouri 64157, employees were exposed to struck by head injuries from working underneath of a powered industrial truck with a makeshift work platform without the use and protection of protective helmets.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  On August 8th 2018, at a residential job site located at 8215 Northeast 90th Street Kansas City, Missouri 64157, employees were exposed to serious caught-in, crushing and or struck-by injuries, in that the forklift operator had not completed a competent training program.

29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.  On August 8th 2018, at a residential job site located at 8215 Northeast 90th Street Kansas City, Missouri 64157, Juan Manuel Garcia equipped a GRADALL 534D9-45 powered industrial truck with a front-end attachment other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachment and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally  centered. Employees were exposed to serious injuries which included struck by hazards from the use and operation of the unauthorized front-end attachment.

On August 8th 2018, at a residential job site located at 8215 Northeast 90th Street Kansas City, Missouri 64157, employees were exposed to crushing and struck by head injuries by being allowed to stand or pass under the elevated portion of any truck, whether loaded or empty.
