On August 8th 2018, at a residential job site located at 8215 Northeast 90th Street Kansas City, Missouri 64157, Juan Manuel Garcia equipped a GRADALL 534D9-45 powered industrial truck with a front-end attachment other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachment and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. Employees were exposed to serious injuries which included struck by hazards from the use and operation of the unauthorized front-end attachment.
