29 CFR  1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:  On or about August 24, 2023, a portable aluminum extension ladder was not inspected for visible defects. The defective ladder was erected with its right side rail bent outwards at approximately 45 degrees.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about August 24, 2023, employees engaged in roofing activities 6ft or above without fall protection. Such activities were applying felt paper and carrying stacks of shingles onto a residential roof.    Manuel Garcia dba Manuel Garcia was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1631182, citation number 1, item number 1 was issued on March 2, 2023, and was affirmed as a final order on May 19, 2023, with respect to a workplace located at 380 E Pleasant St. Oconomowoc, WI 53066.
