29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a)  Hernandez Construction located at 14993 Quince Court, Thornton, CO 80602: On and preceding 8/11/2018, the employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  One employee was installing trusses on a residential roof using a pneumatic nail gun without the use of eye protection.  This exposed the employees to struck-by injuries to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a)  Hernandez Construction located at 14993 Quince Court, Thornton, CO 80602: On and preceding 8/11/2018, the employer did not ensure employees engaged in residential construction activities 6 feet or more above lower levels were protected by guardrail systems, safety net system, or personal fall arrest systems, nor were employees provided with an alternative means of fall protection.  Five employees were securing trusses in place on a residential roof without the use of fall protection . The employees did not have a harness and anchors were not installed.  This condition exposed the employee to falls from height.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a)  Hernandez Construction located at 14993 Quince Court, Thornton, CO 80602: On and preceding 8/11/2018, The employer did not ensure that ladders used to access an upper landing surface had side rails extended at least three feet above the upper landing surface.  Five employees were using an extension ladder to access the roof with approximately 13 inches extending above the landing surface and was not secured.  No other means for accessing the roof was made available.  This condition exposed the employee to falls from height.
