29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Employees were exposed to pinch point/crushing hazards and amputation injury while working on and around the unguarded molding rollers of the IMA Edge Bander (Type Quadromat F 4012). (b) Employees were exposed to pinch point hazards and amputation injury while working on and around the unguarded conveyor return area on the end of the IMA Edge Bander (Type Quadromat F 4012).
