29 CFR 1910.212(a)(2): Guards were not affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. On or about July 24, 2019 the employer did not ensure that guards were secured to the Sheffield vinyl cutting machine, which exposed employees to amputation hazards. The machine did come with a removable guard which slid into place. The guard also had an interlock, however the interlock was setup so that the guard could be lifted approximately one inch before it shut the machine down. Due to the guard not being secured an employee suffered an amputation injury.
