29 CFR 1910.28(b)(11)(ii): (ii) Each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as follows: (see TABLE D-2)    (a) Hill Air Force Base at 6057 Box Elder Lane Building 1285: On or before August 15, 2018, the employer did not ensure that stair rail systems had been installed on all necessary stairways. The stairway on the exterior northwest corner of building 1285 is missing a stair rail on one side of the stairs. This exposes employees to a fall of approximately 3 feet from the open side of the stairs.     (b) Hill Air Force Base at 6025 Dogwood Avenue Building 1286: On or before August 15, 2018 the employer did not ensure that stair rail systems had been installed on all necessary stairways. The stairway on the exterior south side of building 1268 is missing stair rails on each side of the stairs and an intermediate stair rail located in the middle of the stairs. This exposes employees to a fall of approximately 4 feet from the open sides of the stairs.     (c) Hill Air Force Base at Duke Lane Building 1248, On or before August 15, 2018 the employer did not ensure that stair rail systems had been installed on all necessary stairways. The stairway on the exterior southwest corner of building 1248 is missing a stair rail on one side of the stairs. This exposes employees to a fall of approximately 4 feet from the open side of the stairs.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):  (a) Hill Air Force Base at 6057 Box Elder Lane Building 1285: On or before August 15, 2018, the employer did not ensure that all pulleys with parts seven feet or less from the floor were guarded. Pulleys seven feet or less from the ground in the North Mechanical Room did not have guarding to protect an employee from physically contacting the pulley. This exposes employees to potential caught in hazards when performing work in that area.

29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs seven feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt:  (a) Hill Air Force Base at 6057 Box Elder Lane Building 1285: On or before August 15, 2018, the employer did not ensure that all rotating belts in the facility were guarded. A rotating belt seven feet or less from the ground in the North Mechanical Room did not have guarding to protect an employee from contact with the belt. This exposes employees to potential caught in hazards when performing work in that area.

29 CFR 1910.23(d)(2): The minimum perpendicular distance from the centerline of the steps or rungs, or grab bars, or both, to the nearest permanent object in back of the ladder was not at least 7 inches (18 cm), except for elevator pit ladders, which have a minimum perpendicular distance of 4.5 inches (11 cm);    (a) Hill Air Force Base at Connecticut Drive and Aspen Avenue Building 1294: On or before August 15, 2018, the employer did not ensure that the minimum distance from the fixed ladder rungs on the southeast exterior corner of the building was at least 7 inches away from the nearest permanent object of the structure. The perpendicular distance from the 2nd to top rung to the gutter of the building was approximately one inch.

29 CFR 1910.29(f)(1)(ii)(A): The height of stair rail systems installed before January 17, 2017 was not less than 30 inches (76 cm) from the leading edge of the stair tread to the top surface of the top rail:  (a) Hill Air Force Base at 6057 Box Elder Lane Building 1285: On or before August 15, 2018, the employer did not ensure that the stairway located on the exterior west middle side of the building had stair rails that measured not less than 30 inches from the leading edge of the stair tread to the top surface of the top rail. The bottom two treads of the staircase measured approximately 26 inches in height to the top of the stair rail.  (b) Hill Air Force Base at 6057 Box Elder Lane Building 1295: On or before August 15, 2018, the employer did not ensure that two different stairways located on the exterior east middle side of the building had stair rails that measured not less than 30 inches from the leading edge of the stair tread to the top surface of the top rail.  The distance from the foot of the tread to the top of the stair rail was between 22-26 inches on each tread.
