29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems: a. On or about August 20, 2018, at the above addressed jobsite, an employee performing commercial roofing repair operations, nine (9) feet seven (7 ) inches above the ground, was not protected from falling, thereby exposing the employee to fall hazards. Kos Remodeling, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1203705, citation 2, item number 1 and was affirmed as a final order on March 23, 2017 with respect to a workplace located at Lot 86 Michigan Island Dr., Frankfort, IL. Kos Remodeling, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1156677, citation 1, item number 1 and was affirmed as a final order on July 13, 2016 with respect to a workplace located at 4446 Wilson Avenue, Downers Grove, IL.
