29 CFR 1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, shall be conducted as required by paragraph (l)(4)(ii) to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely. a. On or about March 25, 2025, at the above addressed jobsite, an employee was required to operate a powered industrial vehicle and had been provided with training that expired in 2017, including but not limited to formal instruction, practical training and an evaluation of the operator's performance, thereby exposing employee to struck by and crushing hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. On or about February 24, 2025, at the above addressed jobsite, an employee performing residential carpentry operations, twenty-one (21) feet above the ground, was not protected from falls, thereby exposing the employee to fall hazards. KOS Remodeling, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number 1798705, citation number 1 item number 1 and was affirmed as a final order on March 6, 2025, with respect to a workplace located at 133 South Park Ave., Hinsdale, IL. KOS Remodeling, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1477385, citation number 2, item number 1 and was affirmed as a final order on July 27, 2020, with respect to a workplace located at 225 E.
