29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.    At a residential construction site located at 2409 SE Ridge Line Dr., Blue Springs MO: Employees were exposed to electrical shock hazards. The employer failed to protect employee from electrical shock in that equipment was being used with visible insulation damage in multiple areas. This was demonstrated when a DEWALT brand circular saw (Serial # DWE575SB)was connected to an extension cord, and was visibly damaged in multiple areas. Internal wiring was exposed that could have resulted in internal wire damage thus creating a path to ground that was not permanent, continuous or effective.
