29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices:    On or about August 27, 2018, at the dust collector, the employer failed to ensure potential energy was rendered safe. The employer did not have in place energy control procedures to stop the paddle wheel from turning from the weight of the sawdust in the hopper. Employees exposed to an amputation hazard from being struck by the unexpected turning of the paddle wheel blade from the gravitation force of the sawdust in the hopper.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about September 5, 2018, employees were utilizing Gantry Drills (#2, #3, #4) to make boxes and pallets without the rotating drill bits being guarded. Employees exposed to being struck-by the rotating drill bits.

Employees gain access to the disconnect box to isolate the energy sources for the Gang Rip Saw under the company's energy control program.

The junction box near the table was missing the cover plate for the 110 volt wiring. Employees exposed to the hazard of an electrical shock.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:     On or about September 5, 2018, on the shop floor, there were two 110 volt pedestal fans that were not grounded. The fans were being operated and were missing the grounding pins. Employees exposed to the hazard of an electrical shock.
