29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about September 5, 2018, employees working at heights of approximately 55' and were not provided with training to recognize or minimize the hazards associated with a fall. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about September 5, 2018, employees engaged in roofing operations on a flat roof were not protected from a fall, exposing employees to a fall of approximately 55 feet. CUSTOM ROOFING CONTRACTING, LTD. was previously cited for an violation of an equivalent Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1232301, citation number 1, item number 1 and was affirmed as a final order on August 1, 2017, with respect to a workplace located at 3001 Lexington Ln. Glenview, IL 60025 CUSTOM ROOFING CONTRACTING, LTD. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 995173, citation number 1, item number 1 and was affirmed as a final order on October 9, 2014, with respect to a workplace located at Lot#36 Killdeer, IL 60010.
