29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about August 29, 2018, an employee received a right index finger amputation while operating a Landis Threading Machine and the employer did not ensure guarding was installed to eliminate amputation hazards near the point of operation. The employer failed to ensure that the Hepatitis B vaccine and vaccination series were available and offered to employees. There is no abatement certification or documentation required for this item.
