29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. Alkon Corporation - Fremont, Ohio: On or about August 14, 2018, employees were exposed to injury from pinch points and rotating parts in that the 1007 Davenport #2 machine was not locked out prior to setup activities. The employer did not ensure that energy control application steps to control the electrical/mechanical energy were implemented in that a lockout device(s) was not affixed to the energy isolating device followed by the step to verify equipment isolation. b. Alkon Corporation - Fremont, Ohio: On or about September 5, 2018, employees were exposed to injury from pinch points and rotating parts in that the 1014 Davenport #8 machine was not locked out prior to cleaning activities. The employer did not ensure that energy control application steps to control the electrical/mechanical energy were implemented in that the isolating device was not operated, a lockout device(s) was not affixed to the energy isolating device, and verification of isolation was not conducted.

Alkon Corporation - Fremont, Ohio: On or about September 6, 2018, employees that operated the 1014 Davenport #8 were exposed to injury from pinch points and rotating parts created by the machine's moving parts such as, but not limited to, the cam and auger. The machine was not adequately guarded in that employees were not prevented from accessing the hazard areas during operation. b. Alkon Corporation - Fremont, Ohio: On or about September 6, 2018, employees that operated the 1016 Davenport #9 were exposed to injury from pinch points and rotating parts created by the machine's moving parts such as, but not limited to, the cam and auger. The machine was not adequately guarded in that employees were not prevented from accessing the hazard areas during operation and when utilizing the machines controls. c. Employees were not trained to control the machine's hazardous energy during the servicing tasks. Alkon Corporation was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1049237, citation number 1, item number 2 and was affirmed as a final order on September 9, 2015, with respect to a workplace located at 728 Graham Dr., Fremont, Ohio 43420.
