The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.

On or about September 12, 2018, employees were exposed to a fall of approximately 19.5 feet while installing roofing materials on a residential home located on S/L 57 Meadowview, Amherst, Ohio 44001. The employees were working on all areas of the roof and the employer did not ensure that they were trained on how to recognize and minimize fall hazards.

On or about September 12, 2018, employees were exposed to a fall of approximately 19.5 feet while installing roofing materials on a residential home located on S/L 57 Meadowview, Amherst, Ohio 44001. The employees were using an extension ladder to access the roof that did not extend at least three feet above the upper landing area.
