29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:  a) On or about September 13, 2018, at the above referenced inspection site, employees engaged in residential roofing activities were exposed to being struck in the head by falling objects, including tools and roofing materials from upper working levels. The employer failed to ensure that hard hats or other means of protection were used.   There is no abatement certification of documentation required for this item.

29 CFR 1926.102(a)(2): The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects:  a) On or about September 13, 2018, at the above referenced inspection site, employees engaged in residential roofing activities were exposed to injury while operating pneumatic nail guns without eye and face protection. The employer failed to ensure employees wore safety glasses, or other means of protection.   There is no abatement certification of documentation required for this item.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system.  a) On or about September 13, 2018, at the above referenced inspection site, employees engaged in residential roofing activities were not protected from fall hazards. The employer failed to ensure employees wore Personal Fall Arrest Systems (PFAS), or deployed other means of protection. Employees were thereby exposed to injury from falls of approximately 9 feet.     There is no abatement certification of documentation required for this item.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) On or about September 13, 2018, at the above referenced inspection site, employees engaged in residential roofing activities were using portable ladders to access a rooftop approximately nine (9) feet high. The employer failed to ensure that the ladder extended at least three feet above the roof surface. Employees were thereby exposed to injury from falls.     There is no abatement certification of documentation required for this item.
