29 CFR 1910.178(l)(6): 1910.178(l)(6): Located at Defense Commissary Langley AFB, 5100 Spaatz Drive Langley AFB, VA 23665, the employer had not certified that the industrial truck operator had been trained and evaluated in the use of powered industrial trucks as required by paragraph (l) of 1910.178, such as, but not limited to, employee operating the Yale Lift powered electric pallet truck. (a) On or about 09-10-2018 and times prior thereto at Defense Commissary Langley AFB, 5100 Spaatz Drive Langley AFB, VA 23665 , The employer did not certify that a "Marva Maid" subcontractor employee who was operating a Yale Lift Truck, Model #MPW050-EN24T2743, Serial # C802N06681M, Type "E" 24 Volt Electric Truck was trained or evaluated as being competent to do so. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: (a) Date of Violation: On or about 09-10-2018 and times prior thereto, located at Defense Commissary Langley AFB, 5100 Spaatz Drive Langley AFB, VA 23665, employee(s) were exposed to electrical shock hazards from a damaged extension cord with missing outer insulation and exposed conductors that was wrapped around a metal railing and being used to charge the onboard battery charger of a scissor lift. Located in the maintenance area of the warehouse in the battery charging area. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
