29 CFR 1926.501(b)(1): The employer did not ensure each employee on a walking/working surface with an unprotected side or edge which was 6 feet or more above a lower level was protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Employees were exposed to fall hazards when performing top deck work and framing of a structure. The edges of the top deck were not adequately protected. Employees were viewed working on the frame near the edge. Deck height was approximately 15 feet. These conditions were noted on or about 09/26/2018.

A hole in the deck was unprotected. Hole dimensions were approximately 30 inches by 36 inches. Deck height was approximately 15 feet. These conditions were noted on or about 09/26/2018.
