29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:          a) On or about October 2, 2018, employees were exposed to the hazard of falling approximately 18 feet where a portable extension ladder did not extend above the landing surface of a roof deck.         In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):               a) On or about October 2, 2018, employees were exposed to the hazard of falling while performing roofing work without fall protection from a second story pitched roof surface having a ground to eave height of approximately 18 feet.       Paragon Roofing Services Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 1185603, citation 2, item 1, and was affirmed as a final order on February 14, 2017, with respect to a worksite located at 1142 Asbury Ave., Evanston, Illinois.                   No abatement certification or documentation required for this violation.
