29 CFR  1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a)  On October 18, 2023, at the above addressed jobsite, employees conducting gutter installation work from ladder jack scaffolds, approximately 16 feet above the ground, were not provided with guardrails, midrails, cross rails or other means of fall protection, thereby exposing the employees to falls.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
