29 CFR 1915.12(d)(2)(iii): The employer did not ensure that each employee entering a confined space, enclosed space, or other areas with dangerous atmospheres was trained to recognize the adverse health effects that may be caused by the exposure to the hazard. On October 1, 2018 employees worked on the RIG Moccus Vessel located at the Bay Bridge Texas, LLC dock. Employees were required to use respirators to enter and clean 20,000 gallon diesel tanks and were not trained in the recognition of the adverse health effects that may be caused by the exposure to hazardous atmospheres.

29 CFR 1915.75(a): A gangway, ramp or permanent stairway of not less than 20 inches walking surface, of adequate strength, maintained in safe repair and securely fastened, shall be provided between a floating dry dock and the pier or bulkhead. On October 1, 2018 employees worked on the RIG Moccus Vessel located at the Bay Bridge Texas, LLC dock. Employees were required to board this vessel through use of a gangway with damaged rails this exposed employees to fall and possible drowning hazards.
