OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees exposed to fall, stuck-by, and caught under hazards. The employer is failing to protect employees from struck-by and crushing hazards. This condition was most recently occurred on or about October 1, 2018 at 16875 Industrial Park Drive, Bowling Green, Missouri. The employer failed to prevent employee exposure to struck-by and/or crushing hazards due to inadequate bracing of, approximately sixty 57 foot wide, 337 pound prefabricated roof trusses. The six person crew installed trusses on a single-story, wood frame commercial building.

The employer is failing to protect employees from fall, struck-by and crushing hazards. This was most recently documented on or about October 1, 2018 at a jobsite located at or near 16875 Industrial Park Drive, Bowling Green, Missouri. The employer did not establish and implement a program to provide for frequent and regular onsite inspections. A six person crew worked on a commercial construction site where employees were exposed to a variety of hazards to include, but not limited to, fall, struck-by and crushing hazards.

The employer did not ensure employees exposed to fall hazards had been trained by a competent person to properly select suitable anchorage points on roof trusses for personal fall protection. Employees working from the top plate and bottom chord were attaching fall protection with 6 foot lanyards to roof trusses that were not secured or braced.

The employer is failing to protect employees from fall and struck by hazards. This was most recently documented on or about October 1, 2018 at a jobsite located at or near 16875 Industrial Park Drive, Bowling Green, MO. The anchorage points used for attachment of personal fall arrest equipment were not capable of supporting at least 5000 pounds (22.2kN) per employee. Roof trusses, which collapsed, were used as anchorage points exposing employees to fall, and struck-by hazards. Franklin County Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 948959, citation number 2, item number 1 and was affirmed as a final order on 1/7/2014, with respect to a workplace located at 801 Stone Bluff Court, Chesterfield, MO 63005.
