29 CFR 1910.25(b)(5)(i): When a door or gate opens directly on a stairway, a platform was not provided and/or the platform did not provide a useable depth greater or equal to 20 inches: (a) Hill Air Force Base at 6026 Cedar Lane Building 1278: On or before October 10, 2018, the employer did not ensure that when a door opens directly on a stairway, the useable depth was greater than or equal to 20 inches. The doorway located on the exterior north center side of the building was 36 inches wide. When the door was opened, the platform useable depth was reduced from 54 inches to 18 inches. (b) Hill Air Force Base at 6057 Box Elder Lane Building 1285: On or before October 10, 2018, the employer did not ensure that when a door opens directly on a stairway, the useable depth was greater than or equal to 20 inches. The doorway located on the exterior center south side of the building was 36 inches wide. When the door was opened, the platform useable depth was reduced from 53 inches to 17 inches. (c) Hill Air Force Base at 6066 Cedar Lane, Building 1274: On or before October 10, 2018, the employer did not ensure that when a door opens directly on a stairway, the useable depth was greater than or equal to 20 inches. The doorway located on the exterior north side of the building reduces the useable depth of the walking platform to 13 inches when the door is fully opened.

29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as follows: (see TABLE D-2) (a) Hill Air Force Base at 7302 Wardleigh Road Building 15: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. The stairway on the southern section of the east side of the building is missing a handrail on one side of the stairs. The tread width was measured to be 60 inches and no handrail has been placed on the exterior wall of the building. (b) Hill Air Force Base at 7451 6th Street Building 400: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. The stairway on the exterior west side section of the building is missing a handrail on one side of the stairs. The tread width was measured to be 60 inches and no handrail has been placed on the exterior wall of the building. (c) Hill Air Force Base at 7537 Wardleigh Road Building 845: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. Approximately 10 stairways along the exterior north wall of the building are missing a handrail on one side of the stairs. The tread width of each of these stairways was measured to be 60 inches and no handrail has been placed on the exterior wall of the building. (d) Hill Air Force Base at 6069 Wardleigh Road Building 1203: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. The stairway on the exterior south side of the building is missing a handrail on one side of the stairs. The tread width was measured to be 59 inches and no handrail has been placed on the exterior wall of the building. (e) Hill Air Force Base at 6059 Wardleigh Road Building 1204: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. The stairway on the exterior south side of the building is missing a handrail on one side of the stairs. The tread width was measured to be 59 inches and no handrail has been placed on the exterior wall of the building. (f) Hill Air Force Base at Gum Lane, Building 1214: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. The stairway on the western section of the south side of the building is missing a handrail on one side of the stairs. The tread width was measured to be 59 inches and no handrail has been placed on the exterior wall of the building. (g) Hill Air Force Base at 6050 Gum Lane, Building 1215: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. The stairway on the south side of the building is missing a handrail on one side of the stairs. The tread width was measured to be 46 inches from the stair rail to the exterior wall and no handrail has been placed on the exterior wall of the building. (h) Hill Air Force Base at Gum Lane and Florida Street, Building 1222: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. The stairway on the south side of the building is missing a handrail on one side of the stairs. The tread width was measured to be 50 inches and no handrail has been placed on the exterior wall of the building. (i) Hill Air Force Base at Gum Lane, Building 1227: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. The stairway on the south side of the building is missing a handrail on one side of the stairs. The tread width was measured to be 72 inches and no handrail has been placed on the exterior wall of the building. (j) Hill Air Force Base at Fir Avenue and Georgia Street, Building 1231: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. The stairway on the south side of the building is missing a handrail on one side of the stairs. The tread width was measured to be 45 inches from the stair rail to the exterior wall of the building and no handrail has been placed on the exterior wall of the building. (k) Hill Air Force Base at Fir Avenue and Delaware Drive, Building 1233: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. The stairway on the south side of the building is missing a handrail on one side of the stairs. The tread width was measured to be 59 inches and no handrail has been placed on the exterior wall of the building. (l) Hill Air Force Base at Fir Avenue, Building 1236: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. The stairway on the north side of the building is missing a handrail on one side of the stairs. The tread width was measured to be 59 inches and no handrail has been placed on the exterior wall of the building. (m) Hill Air Force Base at Fir Avenue, Building 1237: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. The stairway on the south side of the building is missing a handrail on one side of the stairs. The tread width was measured to be 59 inches and no handrail has been placed on the exterior wall of the building. (n) Hill Air Force Base at 6065 Dogwood Avenue, Building 1264: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. The stairway on the south side of the building is missing a handrail on one side of the stairs. The tread width was measured to be 60 inches and no handrail has been placed on the exterior wall of the building. (o) Hill Air Force Base at 6016 Cedar Lane, Building 1279: On or before October 10, 2018, the employer did not ensure that handrail systems had been installed on all necessary stairways. The stairway on the south side of the building is missing a handrail on one side of the stairs. The distance from the middle rail to the exterior wall was 58 inches and no handrail has been placed on the exterior wall of the building. (p) Hill Air Force Base at 6036 Dogwood Avenue, Building 1277: On or before October 10, 2018, the employer did not ensure that stair rail systems had been installed on all necessary stairways. The stairway on the exterior southeast corner of the building is missing a stair rail on the open side of the stairway. This exposed employees to a fall from approximately 4 feet in height. (q) Hill Air Force Base at 6038 Aspen Avenue, Building 1289 Northeast: On or before October 10, 2018, the employer did not ensure that stair rail systems had been installed on all necessary stairways. The stairway located at the northwest corner of the northeast leg of the building is missing a stair rail on the open side of the stairway. This exposed employees to a fall from approximately 3 feet in height.
