29 CFR 1910.145(e)(2): The wording of signs was not easily read and concise. a) Laminating area Danger signs located on guard for ingoing nip points on Laminator 3 & 4 were not legible and prominently marked. On 10/04/18, an employee cleaning the chrome roller near guard for ingoing nip point crushed four fingers on left hand when hand became caught between the roller and the nip guard. Condition observed on or about 10/11/2018

29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: a) Laminating area Employees used sponges and steel wool to clean chrome rollers near guard for ingoing nip points placing the employees' hands in the danger zone. On 10/04/18, an employee cleaning the chrome roller on Inta-Roto M256 (Laminator #3) next to guard at an ingoing nip point, crushed four fingers on left hand when hand became caught between the roller and the nip guard while rollers were engaged. Condition observed on or about 11/7/2018
