29 CFR 1910.119(h)(3)(i): The contract employer did not assure that each contract employee is trained in the work practices necessary to safely perform his/her job: Employees engaged in preventive maintenance, service, repair activities at the Central Utility Building (CUB) and the Ammonia Engine Room located at Mars Wrigley Confectionery US, LLC. were exposed to chemical hazards associated with the catastrophic release of Ammonia in that the employer did not ensure that the Ammonia Division Service Technicians have been trained on safe work practices such as but not limited to opening process equipment or piping (Line Breaking Policy/Procedure); Hot Work; and Respiratory Protection Program. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.

29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: Employees engaged in preventive maintenance, service, repair activities at the Central Utility Building (CUB) and the Ammonia Engine Room were exposed to chemical hazards associated with the catastrophic release of Ammonia in that the employer did not ensure that the Ammonia Division Service Technicians have been fit tested annually for the 3-M full-face respirators requiring for their tasks. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
