Employees step up and down on the platform on a regular basis throughout the work shift and the employer did not provide standard stairs.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven feet or less above floors or platforms were not enclosed:    On or about October 25, 2018, operator employees at the 808 drop packer line are exposed to the conveyor drive sprocket and chain assembly that was not completely guarded. The side panel of the guarding enclosure was missing near the operator control panel.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions:    On or before October 25, 2018, the 120 volt electrical control panel for the diverter operator at the 808 drop packer line had an electrical fitting with the outer insulation damaged and pulled away exposing the wiring and subjecting the wiring to abrasion.

The 240 volt electrical junction box for the belt motor that was mounted on the wall at the 808 drop packer line had five fitting openings.  b. The junction box for the motor control emergency stop button at the 808 drop packer line had one fitting opening.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    On or about October 25, 2018, an employee was exposed to a machine hazard associated with moving parts and an in-going nip point hazard during a maintenance servicing task while cleaning the bottle conveyor to the drop box assembly at the 808 drop packer line. The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from servicing hazards.     prepare for shutdown [(d)(1)]; - ensure authorized employee has knowledge of the type, magnitude, the hazard, and the method or means to control the energy;  shutdown the machine or equipment [(d)(2)];  machine isolation [(d)(3)]; physically locate and operate all energy isolating devices that are needed to control the energy to the machine or equipment in such a manner as to isolate the machine or equipment from the energy source(s);  LOTO device application [(d)(4)]; affix a lockout or tagout device to each energy isolating device;  dissipation of residual energy [(d)(5)(i)]; relieve, disconnect, restrain, and otherwise render safe all potentially hazardous stored or residual energy;  verification of isolation [(d)(6)]; verify that isolation and deenergization of the machine or equipment has been accomplished;    MALCO PRODUCTS, INC. The 120 volt electrical control panel for the diverter operator at the 808 drop packer line was not secured. The control box was wrapped closed with temporary tape.    b. The 240 volt electrical box for the belt motor that was mounted on the wall at the 808 drop packer line was not secured closed. The two cover fasteners were not secured in place.     c.
