29 CFR 1926.20(b)(2): The employer did not ensure programs were provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:    (a)Xcel Roofing of Siouxland, LLC at 7505 West Boysenberry Street, Sioux Falls, SD 57106: On or about and at times prior to October 31, 2018, the controlling employer did not ensure a safety program was developed and implemented for the work environment to include regular inspections of the job-site, materials and equipment; conducted by a competent person. Subcontractor employees were permitted to work in areas where fall hazards existed without the use of a fall protection system. This condition exposed subcontractor employees to fall hazards greater than 6 feet from a lower level.
