29 CFR 1926.502(f)(1): Warning lines were not erected around all sides of the roof work area: (a)Xcel Roofing of Siouxland, LLC dba Xcel Roofing of Sioux Falls at 3509 West 57th Street, Sioux Falls, SD 57106: On or about and at times prior to December 19, 2019, the controlling employer did not ensure that the warning line system being used for fall protection was erected around the entire work area. Subcontractor employees were working on the roof of a commercial roofing project with inadequate warning lines that did not encompass the work area. This condition exposed subcontracted employees to a fall hazard of 18 feet.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person: (a)Xcel Roofing of Siouxland, LLC dba Xcel Roofing of Sioux Falls at 3509 West 57th Street, Sioux Falls, SD 57106: On or about and at times prior to December 19, 2019, the controlling employer did not ensure that frequent and regular inspections were conducted of the job site. Subcontractor employees were conducting roofing operations and exposed to fall hazards of approximately 18 feet. Xcel Roofing of Siouxland, LLC dba Xcel Roofing of Sioux Falls was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.20(b)(2) , which was contained in OSHA inspection number 1402227, citation number 1, item number 1, and was affirmed as a final order on November 27, 2019, with respect to a workplace located at 509 South Whitewood Avenue, Sioux Falls, SD 57106. Xcel Roofing of Siouxland, LLC dba Xcel Roofing of Sioux Falls was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.20(b)(2) , which was contained in OSHA inspection number 1357211, citation number 1, item number 1, and was affirmed as a final order on December 10, 2018, with respect to a workplace located at 7505 West Boysenberry Street, Sioux Falls, SD 57106.
