29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Plywood Area, 441 West Market Street, Beavertown, PA - The Holzma Optimat HPP 8138 Panel Saw (Serial No. 0-240-15-1043) lacked an effective guard as the hinged blade compartment door was not secured in position to prevent employee access to the rotating main blade and scoring saw blade due to a malfunctioning safety interlock thereby exposing employees to struck by-hazards that could result in an amputation, on or about October 31, 2018.    In accordance with 1903.19, abatement certification is required for this violation.
