29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a) Jobsite: 277 W. Lincoln Way, Exton, PA 19341 - Employees are exposed to fall and struck-by hazards associated with the use of scaffold platforms. The wood framed platform hoisted 24 feet above the ground was not secured to the telehandler forklift to prevent it from moving and falling off the forks. This was condition was documented to occur on or about and at times prior to November 7, 2018.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a) Jobsite: 277 W. Lincoln Way, Exton, PA 19341 - Employees are exposed to fall and struck-by hazards associated with the use of scaffold platforms. A competent person did not inspect a wood framed platform hoisted 24 feet above the ground with telehandler forklift to ensure it was adequately supported and secured. This was condition was documented to occur on or about and at times prior to November 7, 2018.
