29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).   a) Exterior of Building - Back - Roof: Employees without any means of fall protection were exposed to fall hazards of approximately 35 feet while engaged in framing activities , on or about 1/25/19.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:   a) Interior of Building - Extension Ladder to access 2nd Floor: Employees  engaged in framing activities were exposed to fall hazards of approximately 10 feet while utilizing only the upper part of a fiberglass extension ladder to access the 2nd floor, on or about 1/25/2019.
