29 CFR 1910.28(b)(7): 29 CFR 1910.28(b)(7): (7) Openings. The employer must ensure that each employee on a walking-working surface near an opening, including one with a chute attached, where the inside bottom edge of the opening is less than 39 inches (99 cm) above that walking-working surface and the outside bottom edge of the opening is 4 feet (1.2 m) or more above a lower level is protected from falling by the use of: (i) Guardrail systems; (ii) Safety net systems; (iii) Travel restraint systems; or, (iv) Personal fall arrest systems. On or about November 15, 2018, the kettles vertical access hole opening was less than 39 inches from the working platform, kettle deck, and employees were exposed to the access hole opening when adding ingredients to the kettles.

29 CFR 1910.176(b): Storage of material created a hazard: On or about November 15, 2018 incompatible chemicals, acids and basic chemicals were stored together on one containment pallet and accidental mixture of these chemicals may cause an exothermic reaction that may liberate heat and may cause a chemical fire.
