(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:    a. On or about November 6, 2018, an employee who was performing servicing and maintenance on top of a feed mixer was not provided with any form of fall protection, which exposed the employee to a fall hazard of approximately 13 feet.

In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions shall be followed:    29 CFR 1910.147(f)(1)(ii): Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section;    a. On or about November 6, 2018, two employees were performing servicing and maintenance on a Kelly Duplex Feed Mixer, and employees were not removed from the equipment area prior to energizing the equipment for positioning purposes.
